If you are still using the Tramadol, I would suggest stopping it.  It may be doing more harm than good, and since it is a medication usually given on an as needed basis rather than for theuraputic reasons, it is not going to hurt to stop it and see if his behaviour and appetite improve.

I would stop the Tramadol too. If he seems to be in pain you can always start it again. At least you would know if its that. I don't know if the Cerenia stops them from eating, both times Punkin had it he was already not eating. The syringed watered down baby food meat or canned A/D also seemed to get him eating again. Try just a few mls every 2 hours & then try some in a small saucer for him to lick.

Smacking the mouth could be a sign of acid reflux when he puts his head down to eat - common in CRF kitties.  You could try raising his feed bowl so he doesnt need to lower his head.  Assuming the enema cleared all the constipation, has he eaten enough since to get blocked again?
